Region 3B Area Agency on Aging has specially trained and certified resource specialists available from 8:00 am to 4:30 pm, Monday through Friday to answer questions and provide information on local services for older adults, persons with disabilities and family caregivers.

Care Management
I want to stay in my own home, but sometimes I need a little help!
What is care management?
We know that older adults and individuals with disabilities want to stay at home and as independent as possible for as long as possible. Care management (CM) can help prevent or delay the premature or inappropriate admission into a nursing home through services and support that promote continued independence in your own home or that of your caregiver.
MI Choice Waiver Program
The MI Choice Waiver program provides in-home services for low-income, Medicaid-eligible adults who are medically frail and whose care needs would otherwise require nursing home care. Individuals enrolled in the MI Choice Waiver program must meet the requirements for Michigan Medicaid nursing facility level of frailty as determined during a personal in-home assessment, also called a Level of Care Determination.
Eligible adults who meet the income and asset criteria can receive Medicaid-covered services like those provided in nursing homes, but they can stay in their own home, the home of a caregiver, or another residential setting, like an adult foster care, homes for the aged or assisted living facility. All MI Choice Waiver program services are provided free of charge.
Veteran's Directed Home and Community Based Care Program
The Veterans Directed Home and Community Based Care program is a collaborative partnership between local Area Agency’s on Aging, the Veteran’s Health Administration, and the U.S. Department of Veteran’s Affairs. The program provides services to veterans to help them remain in their home or the home of a loved one, with the support of paid caregivers and purchased services such as chore services, respite care and personal care.

Transportation Assistance
Region 3B AAA works in collaboration with several community partners to fund and/or provide accessible transportation to medical appointments, shopping, and volunteer work. Transportation services are coordinated through Community Action's Central Dispatch office for Calhoun County.

Medicare/Medicaid Benefits Counseling
Region 3B AAA provides free MMAP Benefits Counseling to assist seniors in Calhoun County with Medicare, Medicaid, Part D and other insurance needs. Benefits Counselors have been trained in Medicare, Medicaid, and other insurance products, to assist seniors who are unaware of programs they may be eligible for such as, the Medicare Savings Program to pay their Medicare Part B premiums. Some of these programs also help to cover the deductibles for both Part A and Part B. Through the counseling process for Part D, seniors have the opportunity to learn about other beneficial programs that are not well publicized. Counselors provide unbiased education and assistance and are not connected with any insurance company. Call 1-800-803-7174 to speak to a counselor to schedule an appointment.
Elder Abuse Prevention
Region 3B Area Agency on Aging has been instrumental in the development of an elder abuse coalition with a goal of increasing community awareness, coordination of prevention and education activities, and increase reporting of elder abuse in Calhoun and Barry counties. The Calhoun County Elder Abuse Prevention Coalition has grown to more than 40 members representing financial services, senior service providers, law enforcement, adult protective services, advocacy organization, legal services and private sector business leaders.
Thousands of cases of elder abuse are likely to occur each year in Calhoun County, like any other community of our size. Shocked? Almost everyone is. Elder abuse often goes unreported. In fact, only a small fraction of elder abuse incidents are reported each year, leaving the victims at the mercy of their abusers.
